PTO Policy

Time Off Policy: What You Need to Know

When anyone will be away from work for reasons of health, being sick, family matters, or other personal reasons, everyone should inform their team lead and initiate discussion as to if and how missed work can be made up.

For planned PTO, the students need to request PTO at least 4 weeks prior to the first day of planned PTO.

For unplanned PTO, the students need to inform the team lead, Dr. Agarwal, and Fadime ASAP and
complete the PTO as soon as convenient.

Planned PTO includes personal time off, travel, medical (or otherwise) appointments, etc. Unplanned
PTO includes health matters, sick time, family matters, etc. Please check in with Dr. Agarwal if you are in doubt.

COSMOS Research Center Management controls the timekeeping log for every submission and
ultimately, it is at Dr. Agarwal’s discretion as to how absences will be handled, whether approved or not.

Under > Employee Onboarding Checklist > Arrival checklist > 8th bullet point paragraph > click on the hyperlink paid time off (PTO) to download the form and follow the instruction below:

1) Fill out the form & attach the supporting document(s) (if applicable)
2) Have your team lead sign the form.
3) Then send the signed form to Dr. Agarwal (nxagarwal@ualr.edu) for his final approval.
4) Once Dr. Agarwal approves and signs your document, then he will return it to you with the signed copy and Cc Fadime.
5) Then Fadime will document your PTO & Updates Log

This is the process in steps for the PTO request policy for COSMOS Research Center that everyone should be following. Failure to follow departmental notification procedures represents an unauthorized absence and may result in disciplinary action.
